Get emergency care first for these signs

Some dizziness is a medical emergency. Call 911 or go to an emergency room if dizziness comes on suddenly with any of the following:

  • Weakness or numbness in the face, arm or leg
  • Trouble speaking, confusion, or a drooping face
  • Double vision or loss of vision
  • A sudden, severe headache
  • Chest pain, fainting, or a racing or irregular heartbeat

If your dizziness is new and you have not seen a doctor about it, it is worth a medical check before physical therapy.

What vestibular therapy is

Your balance depends on three sources of information working together: your inner ear, your eyes, and the feeling from your feet and joints. When the inner ear part is disturbed, the brain gets mixed signals, and the result can be spinning, a sense of floating, or unsteadiness. Vestibular therapy uses specific exercises and treatments to help that system work together again.

Different kinds of dizziness need different care. Brief spinning when you roll over or tip your head back can come from BPPV, where tiny crystals in the inner ear have moved out of place. Steadier, ongoing unsteadiness often has other causes. Your evaluation is about telling these apart.

What your evaluation includes

Your first visit is a one-on-one evaluation by our Doctor of Physical Therapy. We ask what the dizziness feels like, how long each episode lasts, and what sets it off. Then we may check eye movements, positional tests, walking, and standing balance, always with someone right beside you.

You leave knowing what we found, what we recommend, and whether anything points to a physician follow-up.

What treatment may involve

Depending on what we find, your plan may include:

  • Repositioning maneuvers such as the Epley maneuver, which our clinic performs when testing points to BPPV
  • Gaze-stability exercises that train your eyes and inner ear to work together
  • Balance training on different surfaces and with head turns, progressed safely
  • Walking practice, including turns, curbs and uneven ground
  • Strength work for the legs and hips that supports steadier standing
  • Practical tips for getting out of bed, showering and moving around your home more safely

Balance, confidence and falls

Many people with dizziness start avoiding stairs, driving, church or the grocery store because they worry about falling. Balance work is done in a controlled space, with no more than 2 patients per clinician at a time, so you are never practising on your own. The goal is to help you move with more confidence in the places you actually go.
