Exercise therapy is recommended first-line care for hip osteoarthritis and meaningfully reduces pain for many people, sometimes for years. Replacement is a good operation when symptoms outrun what conservative care can manage — but trying strengthening first costs little and often changes the timeline.
That pattern usually points to the gluteal tendons rather than the joint. It responds to progressive loading, and often to changing positions that compress the tendon — such as crossing your legs or sleeping without a pillow between the knees.
Where time allows, yes. Going into surgery with more hip and leg strength is associated with an easier early recovery, and knowing the exercises in advance makes the first weeks after surgery considerably less daunting.
