You can generally start physical therapy in Texas without a referral. The limits are real but narrow — here is exactly how it works.
One of the most common reasons people delay physical therapy is the belief that they need a doctor’s appointment first. In Texas, that is usually not true — and the wait for the appointment they think they need is often longer than the problem takes to become chronic.
What direct access means
Direct access means a licensed physical therapist can evaluate and treat you without a physician referral. All fifty states permit some form of it. At Direct PT, that means up to 30 consecutive calendar days of care without a referral, starting with an evaluation and plan of care by our Doctor of Physical Therapy.
The limits that apply in Texas
The rules are specific and worth understanding rather than guessing at:
- Your evaluation and plan of care are done by our Doctor of Physical Therapy (DPT)
- Care without a referral runs for up to 30 consecutive calendar days
- If your recovery needs longer, you will need an order from your doctor to continue
- Certain patient populations and clinical situations require physician involvement
- A physical therapist must refer you to a physician if your condition falls outside physical therapy’s scope, or if you are not progressing as expected
Insurance is a separate question
This is the part that catches people out. State law governs whether a therapist may legally treat you; your insurance contract governs whether the plan will pay. Some plans require a physician referral for reimbursement regardless of what state law permits. The two rules are independent, and both have to be satisfied.

- Loss of bladder or bowel control
- Chest pain, shortness of breath, or other symptoms suggesting a medical cause
Part of a physical therapist’s job is screening for exactly these presentations. If something in your evaluation suggests a medical rather than a mechanical problem, you will be told and referred the same day.
